Follow mailing instructions on your bill.  Please include patient’s name with mailed check.

Pay Online with Square 

1. Click the corresponding link below for the service that you would like to make payment for.
2. Enter the information in the required fields:
   • For Therapy – Patient First & Last Name and their Therapist’s Name.
   • For Testing – Patient First & Last Name
3. Complete the Payment Form to make a payment using your credit or debit card.
4. Remember to include Patient First & Last Name or we will not know which account to credit the payment to. Thank you!
